Grant and SPF Forms

1)    SPF Grant Application Form for over £5,000 - this has been updated after working with the Trust and includes some new required signatories which will make the process post-award smoother.

2)    Fund Amendment Form - for removing, adding and replacing fund advisers. Please aim to have 3 up to date nominated fund advisers.

3)    Expenses Claim Form - to be supported with receipts and explanation of them.

4)    Purchase Requisition Form - for non-medical purchases to include supplier quote.

5)    Purchase Requisition Form - for Medical Equipment to include supplier quote. Please note this form must first be submitted to Medical Physics to do their checks and balances who will then submit to us.

6)    Payment Request Form - to be seldom used for direct payment to supplier.

7)    General Grant Application Form - this form is to be used when seeking funds for which SPF funds do not exist.

8)    SPF Annual Review Form – please return this to us. We intend to help you all by sharing ideas in a forthcoming newsletter so this information will be very helpful for that.

9) Grant Impact and Evaluation Form – for those of you with funding approved as a formal grant, please complete this as per the grant terms and conditions.

10) Payment run dates - for fund advisers to be aware of cut off dates and BACS processing dates.
