This project focuses on reducing isolation and improving mental health, particularly amongst groups with the greatest levels of need in the London Borough of Croydon using six locality based interdisciplinary teams.
Kingston & Richmond
The borough of Kingston and Richmond is developing a Proactive Anticipatory Model (PAC) that will target and support people with rising health and social. Targeting people/families that are not usually on the radar of traditional health and care services and would therefore be reactively supported by traditional services at escalation point - the initiative aims to prevent that.
The Primary Care Networks (PCNs) covering the east of the borough (North Merton PCN, Morden PCN and East Merton PCN) are working with local community groups that support underserved community groups to co-design health interventions to improve outcomes for people at risk of or living with diabetes and maternal and new-born health outcomes.
Focusing on tackling the issues in the ethnically diverse and income deprived St Helier ward, which has the highest hospital stays in stays in the borough for alcohol related harm and self-harm, as well as high rates of obesity, depression and diabetes.
This project involves training faith, community and voluntary services to make early intervention healthcare services more accessible for residents of Wandsworth, after evidence showed residents in Wandsworth do not access support until they require acute services or find navigating the system alone difficult to do.
The grant was awarded to the charity in March (2021) and the initiatives/projects are currently on going, in depth updates will follow in the coming weeks.
