Packs for Elderly Care Patients Helping Discharge Process

For many elderly patients, the hospital offers vital social interaction and security, making the transition home feel daunting, even when they are medically ready. This anxiety can sometimes lead to avoidable readmissions. The Juliet O Foundation’s mission is to ease this transition by reducing isolation and supporting their return to everyday life through thoughtfully prepared discharge packs.
The unique packs are filled with essential items such as toiletries, tea, coffee and comforting items such as non-slip socks to assist with a comfortable transition back into the community until the individual regains physical independence, alongside the support of loved ones, social care, or care providers.
After over 35 years of experience working on the frontline of the health and social care sector and motivated by her profound passion for assisting vulnerable adults, Juliet Omoregie realised her dream and launched the Juliet O Foundation. Speaking about the packs, Juliet said: “We believe that these discharge packs can make a real difference in the recovery process for elderly patients. Our goal is to provide comfort and care as they transition home, and we are thrilled to partner with St George’s Hospital Charity to deliver these to those who need them most.”
The Juliet O Foundation has been working with St George’s Hospital Charity, making St George’s the first hospital to provide these packs on their elderly care wards. With the support of St George’s Hospital Charity, who fund and support innovative projects across the hospital that enhance patient care, over 100 patients a month will benefit from the boxes.
The initiative has received generous backing from Juliet’s son, South London-born rapper Dave, who has shown his support by funding the initiative. His contribution ensures that more elderly patients at St George’s can benefit from the comfort and care these discharge packs provide at a critical time in their recovery.
Farisha Agraviador, Head of Nursing for Senior Health at St George’s Hospital, added: “Returning home after a hospital stay can be challenging for many of our elderly patients. These discharge packs offer a thoughtful gesture of support, easing the transition and giving patients what they need to feel more secure and comfortable in those first few days of recovery.”
The initiative has been made possible through the generosity of donors to St George’s Hospital Charity. Every discharge pack serves as a reminder of the kindness that fuels the work of the hospital charity, helping to improve the lives of patients when they need it most.
